That’s the beauty of the plan, if both medics are mafia, then they won’t kill Sakura Oogami anyway because they don’t want the leaders role revealed.

You’re assuming that Swimmer is civilian. If both Medics and the Swimmer are Mafia then they can kill Oogami without any downside.
