No. That is how she died–she was chopped to bits.
She’s been revived, and with that, I’m assuming the memories of their death were forgotten. From what we’ve seen in this chapter, it looks like when they remember their death, their bodies return to the state they were in when they died.

The anime diverges abruptly from the manga especially near the end and a lot of details are left out; the manga is well worth the read.
