Sex and gender are two different things. Sex is how your born, gender how you identify yourself. Gender gets tricky because these vary by cultures, with one culture that i’ve heard of (not studied or read up on them so I can’t give specifics on how they work) has 5. In this case if Kayo identifies by female then good for him/her.
Everyone noticed it, not directly but indirectly… there’s an “human perception curve” (don’t remember how it is called, but you don’t feel the same when you see a robot, a replicant, a man, a girl, etc…) represented on a trend, watching a shemale is the same feeling as watching a replicant, you can notice something is wrong, if you see Kayo’s smile, looks creepy, you feel strange, so you notice something is wrong with her, the same happens with all shemales, because a man can’t be a woman 100%.

That’s why it is differentiated between sex and gender. You are gonna be born a certain way but can later choose to identify for XYZ reasons (could be cultural or biological) as something else. I’m not particularly big on gender issues. Just think of it this way: sex = how you are born, gender= how you identify yourself. The former is strictly biological the latter is cultural/social.
Gender is not my area of expertise but if you want more info on this, I’ll be more than happy to dig around some of my professors for some good reading on the subject.

A sex change will make your appearance better reflect the gender you identify with, but your biological sex will remain the same. A sex change isn’t going to let men bear children or let women impregnate each other.

That depends on whether or not Kara is post op. In order to get there you need a psych evaluation that eventually leads to a green light on the procedure and then, after all is said and done, you would be putting down female. At least, on the legal side of life that’s how it works.
