After buying all the common parts (buttons, acrylic case, harness, etc.) I wired up the buttons, plugged in a USB A to B cable and attempted to use it. When I plug the device in, it connects and immediately disconnects. This happens on Windows 10, Xbox 360, PlayStation 4 and an Ubuntu laptop.
PC is where I get the most data. It plays the connect chime and then the disconnect immediately. The device then acts as though it is mostly non-present. This prevents me from upgrading the firmware.
I’ve RMA’d the board once and the second board has the same issue. Because it is unlikely that I got two defective boards back to back, I assume that I have made some mistake, but I am unsure where to look.
Take clear pictures of your wiring job, and we might be able to help point out where the issue might lie.

I’m actually kind of embarrassed. I went home to take the pictures and I realized that the wires (and particularly the ones used to select consoles) hadn’t been stripped enough to create a good connection in the terminal block. If you plug in the UFB without selecting a platform (at least, on my initial firmware version), the board will immediately disconnect. After fixing the fact that the wires weren’t exposed enough, platform selection works and the problem went away.
