Its translated from Japanese and I only read one scan but its possible that in the that the original text doesn’t say that Kenpachi gave her the scar. Actually the mangastream one doesn’t say he gave her the wound, it just that the would is screaming his name and that it is the only would she has ever recieved. Also the timeline and backstory seems to suggest that she got it from Yamamoto.
From a narrative perspective it doesn’t make sense at all that a wound made by Yamamoto would scream Kempachi’s name.
Also keep in mind that everything has been set to give both of them a background story together.
For example, Ken give to Yashiru her name from the only person he admired which turned to be Unohana.
Some thoughts on this whole topic…
- Look at their relative scars. If Kenpachi is a baby trying to breast feed, I doubt that he’d simply have a face slash while she has a ‘hole’ in her chest.
- If she’s teaching/training him, then she would be his superior - again reflecting on the scars, that seems backwards if htey sparred before.
- Previous language in the chapter seems as though Old Man Yama beat her and thus forced her into the Gotei 13.
- The death comment doesn’t make sense either. Killing a Captain to train Kenpachi is HORRIBLE resource management after they JUST got rocked. I am fond of hte concept of killing the personalities…but that only makes sense in one direction - you cna kill Unohana’s ‘nice’ personality and in essence have two Kenpachi’s - but you cna’t really ‘kill’ kenpachi’s personality, would making Kenpachi ‘smarter’ or anyt other way actually be detrimental for SS? Anything would be an improvement hahaha.
- I’m pretty sure this will be off-panel training short of maybe a random panel or two. Kenpachi’s reveal will be in combat.

- I think the scar screaming his name is more of a metaphor for her excitement to actually get to cut someone to pieces after all of these years. She probably hasn’t fought since she got it.

It “won’t stop throbbing” because the one that put it there is still alive. She’s talking about Kenpachi. There’s significance in them both saying it before their fight to the death. They gave each other their respective scars in a previous battle, and neither of them will be satisfied until they kill the other one.
It’s not that hard to interpret, really.
