i personally took that language more metaphorically, like one of their current personalities will die. AKA if ken pushes unohana too far the mild mannered healer will “die” and the old blood thursty unohana will return. where as if unohana pushes ken past his limits his muscle-bound dumbass, i’m too dumb to even learn my swords name, persona will die and he will have a similar transformation in personality and ability as that of unohana (prolly not straight to a healer, but learning his zanpaktou, respecting it as an ally or whatever).

From the dialogue of the chapter it is implied that the scar on her chest was made by kenpachi while the scar on ken’s face was made by unohana
